Copilot code review adds Max effort level in public preview

Max joins Lite and Balanced as a third review effort level and runs several independent review agents across models from different providers. Copilot approvals are now generally available, off by default, and available on Pro, Pro+, Max, Business and Enterprise plans.

Who is affected:

Copilot code review users on paid plans, and admins who set review effort and approvals.

What to do:

Select Max for high-risk pull requests, and have admins turn on approvals where needed.
